The following steps outline how a Project Access Dallas patient gets treatment at a hospital and how a hospital becomes involved in Project Access Dallas.

Participating PAD Hospital:

If you are a participating PAD hospital and you want to request a patient to be enrolled into PAD, CLICK HERE to download a PDF version of the enrollment form.

After you have completed the form, fax to PAD at 214-946-5805.

Non-participating PAD Hospital:

If you are a non-participating hospital and you want more information on how your hospital can get involved in PAD, please contact Connie Webster, DCMS director of community services at connie@dallas-cms.org or call 214-413-1426.
