Your Role in Getting Well & Staying Well

The Dallas County Medical Society, volunteer physicians, health clinics, pharmacists, hospitals, and many others are volunteering their services to help you get well and stay well. They are not being paid for their services to you. This is not a government program or an "entitlement." Project Access does not cover emergency room expenses, ambulance, or medical supply services. Your responsibilities under this program, the assistance available, and other conditions of the program may change at any time. We reserve the right to check what you have told us and to require that you pay for any assistance you may have received based on inaccurate information you provided.You will be terminated from the program if we find you have intentionally misrepresented information.

General
You agree that you:
1. Will not schedule appointments with any doctor, clinic, or hospital other than the ones to which you have been referred.
2. Will follow your treatment plan; for example, get prescribed medicines and take them as directed.
3. Will promptly supply any information that may be requested by the program.
4. Will allow all information regarding your participation in this program to be shared with other individuals, organizations, and agencies at the discretion of Project Access.
5. Will immediately contact your enrollment site if your income changes or if you become covered by Medicare, Medicaid, CHIP, VA benefits, disability, Parkland HealthPlus, or employer-sponsored insurance.
6. Will apply for Medicaid or other assistance programs at our request.
7. Will go to an emergency room if you have an emergency illness, because Project Access is an outpatient service.

Eligibility
You understand that:
1. Your term of eligibility expires 6 months from the day of enrollment.
2. You must be responsible for calling to receive an extension when your enrollment ends.

Referrals
You agree to:
1. Keep each doctor’s appointment. (If you miss 2 appointments in 12 months without letting the doctor’s office know at least 24 hours before your appointment, you will be dropped from the program.)
2. Present your Project Access ID card each time you see a doctor.
3. Call your enrollment site or doctor if you need to be seen anywhere else for care.

Medications Assistance
You understand that:
1. A prescription that costs more than $250 will not be covered.
2. Pharmacy benefits are limited to $750 each 12-month period.
3. Most types of medications are available through this program. Your physician may be contacted and asked to use medications that are covered by the program.
4. A pharmacy may stop participating at any time, for any reason.
5. You are to present your medications card each time you have a prescription filled.
