Project Access Dallas Enrollment

If you are a participating PAD physician and you want to refer a patient into the program, click here.

Once you have completed the enrollment request form, fax it to PAD at 214-941-3351. When PAD receives the form, they contact the patient to verify whether they qualify for the program or not. If the patient qualifies for PAD, they will be given an enrollment appointment, which will be conducted at DCMS offices. At the enrollment appointment, the patient is oriented to the program; signs consent forms and is given a PAD ID card.

You will be notified when the patient is enrolled in the program. If you are the patient’s PCP, you will remain their PCP. If you are a specialist, the patient will be assigned a PCP. By enrolling your patient into the program, you will have access to all the resources you need to take care of your patient.

In addition to accepting enrollment requests from participating PAD physicians, patients are also enrolled into the program from the following charity medical clinics. These community clinics serve as enrollment sites for PAD.
